OmG im so worried will he need a doctor?

Yes. He needs to get to a doctor ASAP. Chest pain should not be taken lightly. Has he been diagnosed by a doctor with Bronchitis or self diagnosed. It could be COPD, chronic bronchitis, congestive heart failure, etc. If he is having problems in his feet and is overweight he could be diabetic. He could have a lot of things, but he doesn't sound well and needs to see the doctor ASAP. I know he already knows this, but he needs to stop smoking.
